Overview

Insomnia
Insomnia is a common sleep disorder involving persistent difficulty falling asleep, staying asleep, or obtaining restorative sleep despite having an adequate opportunity to sleep. It can cause daytime tiredness, poor concentration, irritability, reduced performance, and other difficulties. Insomnia may be short term or chronic and can occur on its own or alongside medical, psychological, medication-related, or sleep-related conditions.
Polymyalgia Rheumatica
Polymyalgia rheumatica is an inflammatory disorder that causes pain and stiffness, especially in the shoulders, neck, and hips. Symptoms are often worse in the morning or after periods of rest. It mainly affects adults over 50 and may occur together with giant cell arteritis, which can cause headaches, vision problems, and other vascular symptoms.

Diagnosis

Insomnia
Diagnosis is based mainly on a detailed history of sleep patterns, daytime symptoms, health conditions, medicines, and lifestyle factors. A sleep diary may help identify patterns and triggers. Healthcare professionals may use questionnaires to assess insomnia and daytime sleepiness. A physical examination or laboratory testing may be used when an underlying condition is suspected. Sleep studies are generally reserved for cases in which another sleep disorder, such as sleep apnea, is suspected.
Polymyalgia Rheumatica
Polymyalgia rheumatica is diagnosed from the pattern of pain and stiffness, medical history, and physical examination. There is no single test that confirms the condition. Blood tests such as erythrocyte sedimentation rate and C-reactive protein may show inflammation. Additional testing may be used to exclude rheumatoid arthritis, other inflammatory disorders, infections, or malignancy.

Prevention

Insomnia
Consistent sleep and wake times, a comfortable dark and quiet bedroom, regular physical activity, and limiting caffeine and nicotine later in the day can support healthy sleep. Avoiding heavy meals and excessive alcohol close to bedtime may also help. Limiting stimulating activities and bright screens before bed can be useful for some people. Persistent insomnia should be evaluated, particularly when it causes significant daytime impairment or occurs with other symptoms.
Polymyalgia Rheumatica
There is no known way to prevent polymyalgia rheumatica. Because the condition is associated with giant cell arteritis, people diagnosed with PMR should be aware of new headaches, scalp tenderness, jaw pain while chewing, or visual changes and seek prompt medical evaluation if these occur. Regular follow-up can help monitor symptoms and treatment-related complications.

Risk Factors

Insomnia
Risk increases with ongoing stress, irregular sleep schedules, shift work, travel across time zones, older age, and certain medical or mental health conditions. Caffeine, nicotine, alcohol, some medicines, and poor sleep habits can contribute. A previous episode of insomnia may increase the likelihood of recurrence. People who experience persistent pain, breathing problems during sleep, or other sleep disorders may also have increased risk.
Polymyalgia Rheumatica
Polymyalgia rheumatica occurs almost exclusively in adults over 50 and is more common in women. Risk may also be influenced by genetic and geographic factors. Having giant cell arteritis is closely associated with PMR, and people with PMR may develop symptoms of giant cell arteritis. There are no established lifestyle measures known to prevent the disorder.
