Overview

Epididymitis
Epididymitis is inflammation of the epididymis, the tube that carries sperm from the testicle toward the vas deferens. It commonly causes pain and swelling in the scrotum and may also cause urinary symptoms or fever. Infection is the most common cause, although medicines and other conditions can occasionally contribute.
Adrenal Insufficiency
Adrenal insufficiency is a condition in which the adrenal glands do not produce enough hormones needed for normal body function. It may result from problems affecting the adrenal glands themselves or from disorders involving the pituitary or other parts of the hormone-regulating system. Symptoms can develop gradually and may include fatigue, weakness, weight loss, and low blood pressure.

Diagnosis

Epididymitis
Diagnosis is based on symptoms and physical examination, including assessment of the scrotum and testicles. Urine and blood tests may be used to identify infection, and testing for sexually transmitted infections may be appropriate. Ultrasound may be used when necessary, particularly to help distinguish epididymitis from other causes of acute scrotal pain.
Adrenal Insufficiency
Diagnosis of adrenal insufficiency is based on symptoms, medical history, physical examination, and blood tests measuring adrenal hormones. Testing may show low cortisol or abnormal electrolyte levels. An ACTH stimulation test can assess adrenal response. Depending on the suspected cause, additional hormone tests, antibody testing, and imaging such as CT or MRI may be used.

Prevention

Epididymitis
Safer sexual practices, including appropriate condom use, can reduce the risk of sexually transmitted infections that cause epididymitis. Prompt diagnosis and treatment of urinary or sexually transmitted infections can reduce complications. People with urinary tract abnormalities or recurrent infections should follow appropriate medical management and seek evaluation for persistent urinary or scrotal symptoms.
Adrenal Insufficiency
Adrenal insufficiency cannot always be prevented because it may result from autoimmune disease or other conditions that cannot be avoided. Early diagnosis and appropriate treatment can reduce the risk of complications. People taking medicines that may affect adrenal function should follow medical guidance and should not change long-term treatment without professional advice.

Risk Factors

Epididymitis
Risk increases with unprotected sexual activity and sexually transmitted infections, urinary tract infections, prostate or urinary tract abnormalities, recent urinary procedures, and regular use of a urinary catheter. Previous urinary tract surgery or structural problems may also increase risk.
Adrenal Insufficiency
Risk factors for adrenal insufficiency depend on whether the problem originates in the adrenal glands, pituitary gland, or another part of the hormone-regulating system. Autoimmune disorders and chronic illnesses may increase risk. Certain infections, prolonged use of some medicines, previous surgery, bleeding, tumors, or other conditions affecting hormone production may also contribute.
