Overview

Cold Sores
Cold sores are small, painful, fluid-filled blisters that usually develop on or around the lips and mouth. They are caused by herpes simplex virus and often recur after the initial infection because the virus remains inactive in nerve tissue. Outbreaks commonly heal within several days to a few weeks.
Epididymitis
Epididymitis is inflammation of the epididymis, the tube that carries sperm from the testicle toward the vas deferens. It commonly causes pain and swelling in the scrotum and may also cause urinary symptoms or fever. Infection is the most common cause, although medicines and other conditions can occasionally contribute.

Diagnosis

Cold Sores
Cold sores are usually diagnosed clinically from their characteristic appearance and symptoms, particularly grouped fluid-filled blisters around the lips or mouth. A healthcare professional may examine the affected area and ask about previous episodes and possible triggers. A swab or laboratory test may be considered when the appearance is unusual or the diagnosis is uncertain.
Epididymitis
Diagnosis is based on symptoms and physical examination, including assessment of the scrotum and testicles. Urine and blood tests may be used to identify infection, and testing for sexually transmitted infections may be appropriate. Ultrasound may be used when necessary, particularly to help distinguish epididymitis from other causes of acute scrotal pain.

Prevention

Cold Sores
Avoiding direct contact with active cold sores and not sharing items that contact the mouth can reduce transmission. During an outbreak, avoid kissing and oral contact with others until lesions have healed. Identifying and reducing personal triggers such as excessive sun exposure or fatigue may reduce recurrences. People with frequent or severe outbreaks should discuss treatment options with a healthcare professional.
Epididymitis
Safer sexual practices, including appropriate condom use, can reduce the risk of sexually transmitted infections that cause epididymitis. Prompt diagnosis and treatment of urinary or sexually transmitted infections can reduce complications. People with urinary tract abnormalities or recurrent infections should follow appropriate medical management and seek evaluation for persistent urinary or scrotal symptoms.

Risk Factors

Cold Sores
Cold sore outbreaks may be more likely during periods of physical or emotional stress, fever, illness, fatigue, or hormonal changes. Sun exposure and local irritation around the lips can also trigger recurrence. People with weakened immune systems may experience more frequent, severe, or persistent outbreaks.
Epididymitis
Risk increases with unprotected sexual activity and sexually transmitted infections, urinary tract infections, prostate or urinary tract abnormalities, recent urinary procedures, and regular use of a urinary catheter. Previous urinary tract surgery or structural problems may also increase risk.
